What can I expect at my first appointment?

Please bring your insurance card and driver’s license with you. We will collect any deductible or copay, that is due per your insurance before being brought back to the treatment room. The therapist will discuss your pain issues, and perform a full-body evaluation, to help identify your physical limitations. The therapist will create a personalized care plan and set-up a home program in order for you to begin your recovery process. Your appointments will typically last 45 minutes to 1 hour.

Do I need an order or a referral before I can be seen?

In order to bill your insurance for your physical therapy appointment, you will need an “order” from a Doctor, Chiropractor, or Dentist. It can have any physical therapy name or location, it is good where ever you choose to have your care. It is similar to taking a prescription to any drug store of your choice.

A “referral” is required by some insurance plans; however, it is very rare. You will need to confirm with your insurance carrier if your policy requires a physical therapy “referral” from your medical doctor. If it is required, your medical Doctor will need to request, from your insurance company, an authorization for a specific number of physical therapy visits, and it must state Synergy Physical Therapy, Inc. for your care.

We are providers for most insurance and Medicare plans. Please contact your insurance provider to confirm your health care benefits and eligibility.

Do you have cancellation policy?

We understand that unexpected events can occur. Out of consideration for our staff and other clients, who are waiting for an appointment to become available, we have adopted the following cancellation policy:

We appreciate a 24-hour notice for cancellations whenever possible. Missed or cancelled appointments without a 24-hour notice, we reserve the right to charge a late cancellation fee, which is not billable or reimbursed by your insurance.
